Need help with Google adsense?

Need help with Google adsense?
I had created Google adsence account long before and I forget each and every information about it. later on I wanted to created another account but failed. In this way I may be created 3 or 4 accounts using my e-mails account now I need help to restore my account anyhow. That can be only one account that will be counted. and I tried many time to restore but failed. Can any one help with information?
Please if you can give me the full process written. And if you are earning with Google adsense give me some advise.
